ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
  2.  
  3. Develop and revise a Plan of Care in consultation with the physician and other care-team members.
  4. Your position will include: Participation in coordination of patient care. 
  5. Provide therapeutic treatments, evaluate need for and use of adaptive equipment and environmental modifications fabricate/maintain adaptive equipment and hand splints as applicable.                                                                                    ,

  7. Evaluate outcomes of care with appropriate discharge planning.
  8.  
  9. Apply concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  10.  
  11. Accept clinical assignments that are consistent with education and competence to care for patients.
  12.  
  13. Supervise COTA and HHA as appropriate.
  14.  
  15. Complete appropriate documentation in a timely manner.
  16.  
  17. Meet mandatory continuing education requirements of the agency/licensing board.
  18.  
  19. Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
  20.  
  21. Demonstrate commitment, professional growth and competency.

  23. Promote Agency philosophy and administrative policies to ensure quality of care.
