1. Provides appropriate clinical management of the patients assigned to the schedule.
2. Conduct comprehensive OASIS assessments and care plan development.
3. Provide skilled nursing care as outlined in the plan of care.
4. Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
5. Practice knowledge of Medicare CoP's, proficient documentation skills, attention to detail, prioritization, and time management.
6. Utilize effective written and verbal interpersonal communication skills.
7. Evaluate effectiveness of services provided to the patient, consulting with physicians as needed.
8. Submit detailed clinical documentation within expected timeframe.
9. Monitor clients' conditions; reporting changes to Clinical Manager and Physicians.
10. Ensure all personnel involved in client care have current clinical information and current orders.
11. Follow up with, executing, and properly documenting doctors' orders.
12. Case management and care coordination while in the field with support from the clinical manager.
13. Assure that care is in accordance with Federal and State guidelines.
14. Full time minimum visit expectation per established metrics as defined in offer letter. (full-time only)
15. Perform on call rotation as assigned per on-call schedule. (full-time only)
16. Under the direction of the Branch Clinical Manager, participate in planning and implementing specialized procedures and duties that assist in meeting the nursing care needs of the patient in accordance with agency policies.
17. Assist the Branch Clinical Manager in carrying out the plan of care.
18. Assist the patient in learning appropriate self care techniques.
19. Complete appropriate documentation in a timely manner.
20. Provide care utilizing infection control measures that protect both staff and patient (OSHA)
21. Provide effective communication to patient/family, team members, and other health care professionals.
22. Monitor assigned cases to ensure compliance with requirements of third party payor
23. Demonstrate commitment, professional growth and competency
24. Promote Agency philosophy and administrative policies to ensure quality of care
